When you apply for the card you will be asked to fill in a form with information about yourself, including how long you have been residing in Indonesia and some other contact you have had with Avian Flu. Then you'll be asked to provide your contact information, such as friends members and your family the Indonesian Health Emergency staff can contact you if there be a crisis.
