What is the Maldives Health Declaration form?
When applying for a passport, or naturalization, a health declaration form is typically filled out. You may also fill it up when you apply for employment or on retirement. The process of filling up this form is not very complicated and requires only your personal details and a short explanation of your profession. You can also get it from any Maldives medical practitioner's clinic if you wish to fill it up yourself. The health insurance program is administered by the Ministry of Health and is designed to provide healthcare services to its citizens free of cost. All residents of the country are required to have a Maldives Health Declaration Form. This is to ensure that one does not fall sick without knowing his/her current condition or how much the treatment would cost. In case you happen to fall ill or become blind, the form will help the government in knowing these details. It can also be filled out at any Maldives clinic by a medical practitioner. Two sections are found in the form, namely the Personal Information and the Public Health information. The Personal information section includes your personal details such as your name, address, contact details and passport. The Public Health section provides basic information about your past records and current health status. You may have to submit a proof of citizenship or a copy of a birth certificate in order to prove that you belong to the Maldives.
