Form for the Northern Mariana Islands Health Declaration
First, the resident should contact their primary care provider. They should have their primary care provider with them during the entire sign up process. Once the primary care provider has been identified, residents can begin to fill out the health declaration form. There is usually a section on this form that requires the resident to list all current medications as well as all doctors they see on a regular basis. All medication should be documented so that health officials can make a decision as to what type of care is needed for each medical condition. There is also a listing of the illnesses that the resident suffers with. The illness should not affect your ability to live a normal lifestyle. Some examples of conditions are: diabetes, cancer, heart disease, asthma, high blood pressure, obesity, osteoporosis, parasites, sleep apnea, and vision problems. Most people do not need to list any conditions that affect their ability to work or the ability to earn a living. These are only a few of the many conditions that should be highlighted on the document. Once the resident has completed the health declaration, they can forward it along the appropriate fee to the local Board of Health. The fee is used to pay for the services that the Northern Mariana Health Department provides to residents. However, there are often additional costs that the residents of Northern Mariana Island are required to pay. These costs are to cover the cost of providing health care services for residents. This includes immunizations, testing for HIV/AIDS and other infectious diseases like diabetes. These tests can be very expensive and insurance coverage may not always cover them.
